About Dr. Mayeux

The physician behind the opinion.

Dr. Doug Mayeux is a board-certified emergency medicine physician and a disabled veteran of the United States Air Force, where he served as an enlisted weather forecaster before pursuing medicine. He founded Sworn Medical because he understands both sides of the VA disability process: as a physician who evaluates medical evidence, and as a veteran who has navigated the system himself.

Every nexus letter from Sworn Medical is personally reviewed, edited, and signed by Dr. Mayeux. There are no mid-level providers, no physician contractors, and no form-letter templates. Each opinion reflects independent clinical judgment applied to your specific medical records and service history.

The name "Sworn" reflects a simple truth: physicians swear an oath when they enter medicine, and veterans swear an oath when they enter service. Your medical opinion should come from someone who understands both.

A few states are reviewed case by case for physician licensing reasons; if yours is one, you will hear that on the free screening call before anything else happens.

Standard Nexus Letter

Tier 3C: Complex Nexus Opinion
The same opinion, for a claim in a harder posture. Your case is Tier 3C only if one or more of these is true:

  • You are responding to a negative C&P examiner opinion
  • Your claim is at appeal: Higher-Level Review, supplemental claim, or BVA
A secondary claim on its own is a standard letter. Claiming a condition as secondary to one you are already service-connected for does not change the tier unless one of the three triggers above also applies.
These do not affect your price: how many pages of records you have, how difficult the causation question turns out to be, or how long the review takes me. Those are mine, not yours.

Your price is set on the screening call, in writing, before you pay anything. It can change in exactly one situation: if the records analysis turns up one of the three Tier 3C triggers above that was not known on the call (a prior denial you had forgotten, for instance). If that happens, I will tell you before doing any further work, and you can either proceed at the revised tier or stop with the analysis you have already received and owe nothing more. Nothing else changes your price.

And if I cannot take your case for a reason on my end, because it falls outside my scope, because I am at capacity, or because I cannot meet your filing deadline, you will hear that on the call and owe nothing.
Hardship. A fixed portion of my caseload is reserved for reduced-fee and pro-bono opinions for veterans in financial hardship. If that is you, just ask. It will not change how your case is handled.

Fees are for the physician's professional opinion regardless of the VA's decision. Insurance is not accepted. Payment is collected before records review begins.

What to Expect

The process from first call to signed letter.

Request a Call

Fill out a brief form with your name and branch of service. No medical records needed at this stage. Dr. Mayeux replies with a booking link and a short screening form, five questions, that lets him prepare for your call.

The Screening Call (15 Minutes, Free)

Dr. Mayeux walks through the condition, the in-service event or exposure, and where your claim stands. This is an honest conversation: if the evidence is unlikely to support a nexus opinion, he will say so, and you will owe nothing. If it may, he quotes your price in writing before you pay anything.

Upload Your Records

If you decide to move forward, you’ll receive a secure, HIPAA-compliant link to upload your records: DD-214, service treatment records, post-service medical records, any prior VA decisions, and C&P exam reports if available.

Physician Review & Draft

Dr. Mayeux reviews your complete medical record, analyzes the medical evidence, researches the relevant peer-reviewed literature, and drafts a detailed nexus opinion. Every letter is written in his own clinical judgment, no templates, no mid-level providers.

Signed Letter Delivered

You receive a physician-signed nexus letter as a PDF, ready to submit to the VA with your claim. The letter includes cited medical literature, applicable case law, and the standard “at least as likely as not” opinion language that VA adjudicators look for. Standard turnaround is two to three weeks.
